After having a few issues in Hyjal with pulling aggro after 2-3 FDs into a fight and way below the tank for threat on Omen (50% and 30%) I did a little research. Dug up the following thread on the EU forums (http://forums.wow-europe.com/thread.html?topicId=3779200638&sid=1) which has a blue reply and the following thread on the US forums (http://forums.worldofwarcraft.com/thread.html?topicId=3881781552&sid=1) which has no blue response but provides links to EJ and TKA. Don't know if you all have experienced similar issues but it might be beneficial to bump up Salv in blessing priority and use FD constantly every cooldown even when threat is showing low until the issue is fixed.

It's been wierd....it's affected Haythan but not me for the most part, I have had it once or twice but he's suffered a ton from it.

No one has been able to pin it down to any specific cause so its very very hard to re-create and prove. Prior to this I'd had only two possible instances of this happening. Once was on Gurtogg (but a lot of threat dropping on tanks there and had a tank die at the same time) and the other was on the Lynx on a timed mount run (got aggro as soon as I switched back from a totem so wasn't able to say for sure I was low on Omen).

Haythan I think had a issue also with Bloodboil, but I think he has had problems on trash like Behemoths and... Gorefiend too maybe? Can't remember all the times its happened.

Hmm, now that you mention it possibly a couple of the larger trash mobs as well...although I run a little closer on threat for trash so didn't think too much of it.

I just can't recall how many times of I've heard Haythan say "I just feigned, why am I pulling aggro? It wasn't resisted" Since the patch, its been heard often.
